 Child must be age 5 on or before September 1, 2020
 Parent/Guardian must register child
 Proof of Immunization
 Birth Certificate
 3 Forms of Proof of Residency and Driver’s License
1. Lease/Mortgage Statement
2. Utility Bill
3. Pay Stub/Bank Statement
