Requirements:

 Child must be age 5 on or before September 1, 2014

 Parent/Guardian must register child

 3 Forms of Proof of Residency

Legal document i.e. mortgage statement, or lease

payroll stub, car registration, utility bill etc.

 Proof of Immunization

 Birth Certificate
